c. Faster progression of ATT
d. TB more common in AIDS
e. Occurs in early stages of HIV
Ans.
(B) Same susceptibility to ATT; (D) TB is
more common in AIDS.
• Tuberculosis is an important opportumstic disease among
HIV infected individual
• Incidence is about 70 — 80%
• Tuberculosis can appear at any stage of HIV infection
• When cell-mediate immunity is only partially compromised,
Pulmonary tuberculosis present as typical pattern but
in late stage of HIV infection primary tuberculosis like
pattern with diffuse interstitial or miliary infiltrates, little or
no cavitation with intrathoracic lymphadenopathy is common.
• Sputum smear is less frequently
positive.
• Extra pulmonary tuberculosis is about 30-40% among HIV
infected patients.
• Standard treatment regimens are
equally efficacious in HIV negative and HIV positive patients.
• Three important considerations relevant to tuberculosis
treatment in HIV infected patients are:
- An increased frequency of immune reconstitution syndrome.
- Drugs interaction between HAART and rifamycins.
- Development of rifampin mono-resistance with widely spaced
intermittent treatment.
- CPT (cotrimoxazole) to be included in regime for
management
