Glomerular disease | PGI Nov 2017 Preparations MCQ

In glomerular disease which of the following is manly excreted in Urine

A. Albumin.
B. Globulin.
C. Light chain.
D. Heavy chain.
E. Tamm-Horsfall protein

Answer. A. Albumin ;
• Normally glomeruli of kidney does not allow substances with MW >69,000. When they are damaged or
diseased, they become more permeable and plasma proteins may appear in urine.
• The smaller molecules of albumin passes through damaged glomeruli more readily than the heavier
globulin, so albumin predominates in urine.
• Light chains are filtered (MW. 25.000) freely in the glomeruli and appears in urine (eg. Bence John
proteinuria in multiple myeloma).

• Heavy chains are (MW about 50.000) also permeable across glomeruli.
