•
Collection of pus in the uterine cavity is called pyometra. In presence of
pyometra, the uterus is enlarged, soft and tender. The diagnosis may sometimes
be confused with Ca endometrium, as it may be a related feature in both senile
endometritis and Ca endometrium. Diagnostic curettage with endometrial
histology should be performed to confirm the diagnosis. In cases of isolated
pyometra due to infectious etiology, simple dilation with drainage of the pus
should be done.
The
prerequitsites for pyometra are:
-
Occlusion of the cervical canal
-
Enough sources of pus formation inside
the uterine cavity
-
Presence of low grade infections
Causes:
-
Obstetrical: infection of lochiometra
-
Gynecological:
o
Ca uterus (lower part)
o
Endocervical cancer
o
Senile endometritis
o
Infected hematometra
o
Tubercular endometritis
